Is this the disc version or a digital download of Injustice 2?

This rental is the physical PS4 disc. You will need a PS4 console with a working disc drive to play it.

Does the rental include a PS4 console and controller, or just the game disc?

This listing is for the Injustice 2 disc only. Consoles, controllers, HDMI cables and other accessories must be rented separately - WhatsApp +94 777 186 500 for a quote if you need a full setup.

Will my progress and unlocked characters save if I return the disc and rent again later?

Save data is stored on your own PS4 console or PlayStation cloud storage if you have PS Plus. The rented disc itself does not store any progress, so your saves remain on your account.
